I understand that this registration will be accepted upon its submission, and I agree that:
Registration is subject to the rules and regulations of Lakeview Malverne Lions Inc. and any other sports or athletic organizations with which the organization is or may become affiliated.
Registration may be revoked with or without cause at the discretion of the Board of Directors of Lakeview Malverne Lions Inc..
Registration includes limited insurance coverage which is excess insurance only that becomes effective only after another personal insurance coverage has been exhausted.
Registration is subject in particular to the Lakeview Malverne Lions Inc. Player and Parent Code of Conduct, receipt of which is hereby acknowledged.
The registrant/player named below may participate in the programs listed below during the season covered by the application.
